• Name of your primary medical doctor and any previous podiatrists
• List of medications you take including dose, frequency, and allergies

• Past medical records or x-rays (if you have them) relating to your current or previous foot problems

• Insurance card and picture identification

When to arrive:

We value your time. Help us remain on schedule. New patients should arrive approximately 15 minutes prior to their appointment. This provides time to gather necessary information or update records for existing patients who have not been seen within the past year. Your initial visit typically requires up to 60 minutes, depending upon the nature of your problem.
